rx: Add a no-op afs_kcondvar_t for LWP

Remove the need to protect afs_kcondvar_t with RX_ENABLE_LOCKS, by
typedefing it as an int in rx_lwp.c. This is identical to what we
already do with afs_kmutex_t, and should let us get rid of some more
preprocessor spaghetti.
